Berberine is an isoquinoline alkaloid found in goldenseal, barberry, and Oregon grape. It has been used in traditional Chinese medicine for centuries and has emerged as one of the most impressive natural compounds for metabolic health — often compared directly to metformin.
A landmark 2008 study in Metabolism showed berberine reduced HbA1c by 0.9% vs metformin's 1.0% — essentially equivalent. Berberine additionally improved lipid profiles more than metformin. However, berberine has lower bioavailability and shorter duration of action.
Dihydroberberine (DHB) is a metabolically active form with 5x better absorption than standard berberine, allowing lower effective doses and less GI side effects.
Berberine activates AMPK by inhibiting mitochondrial complex I (same mechanism as metformin). It increases insulin receptor expression, promotes GLUT4 translocation, inhibits DPP-4 (similar to diabetes drugs), and modulates the gut microbiome to reduce endotoxemia.
